Medication Policy

St. Thomas More School requests that medication be given at home during non school hours. However, it recognizes that sometimes it is essential for medication to be administered at school.

All medication must be in a labeled container, prescription or over-the-counter.

Prescription medications MUST be in their original pharmacy labeled container, which should include the name and phone number of the pharmacy, the student's name, the physician's name, the medication, the currently prescribed dose, time of administration and Rx numbers.

Medication dated within the last two weeks and to be administered NO MORE THAN 10 SCHOOL DAYS require a parent's/guardian's signature and the completion
of the information on the Request for Administration of Medication During
School Hours
form.

Medication prescribed for longer than a two week period ALSO require the prescribing physician's signature.

Over-the-counter medication requires the prescribing physician's signature. Only prescribed physician's over-the-counter medications will be administered at school.
